Electronic filing system has launched! 2022 Form 6, Form 6X (all years), and 2023 Form 6F must be filed electronically this year. Paper forms will not be promulgated. Agency Coordinators have been emailed an invitation to register for the system and provide their annual update of filers for their agency. Form 6 filers will receive email invitations in March. Form 6 filers needing earlier access may request an invitation to register by calling the Commission, (850) 488-7864.

The forms are called prescribed forms because the wording in them has been carefully drafted to ensure that the person completing the form and the person on whom it is served may understand what is being proposed and what they may do in response. Failure to serve the correct form may invalidate the action that is being proposed.

This form should be used by landlords where possession of accommodation, let under an assured tenancy, an assured agricultural occupancy or an assured shorthold tenancy, is sought on one of the grounds in Schedule 2 to the Housing Act 1988.
This form should be used by tenants when their landlord has served notice proposing a new rent under an assured periodic tenancy, including an assured shorthold periodic tenancy, or to refer a notice proposing a new rent or licence fee for an assured periodic agricultural occupancy.
This form should be used by a tenant with an assured shorthold tenancy which began (or for which a contract had been made) before 28 February 1997, to apply to the local rent assessment committee, during the fixed term of the original tenancy, to have the rent reduced.

This form should only be used by an assured tenant. The tenant should only use this notice to notify their landlord that they wish their assured tenancy to be replaced by an assured shorthold tenancy. Tenants should seek legal advice before completing this form.
In compliance with the National Flood Insurance Program regulations in the Code of Federal Regulations Title 44 Section 65.13, this form is used to ensure structural flood control measures in areas subject to alluvial fan flooding is designed and/or constructed to provide 1% annual-chance or greater flood hazard reduction. This is required for FEMA to recognize it on a flood map.
Elevating a parcel of land or a structure by fill or other means will not serve as a basis for removing areas subject to alluvial fan flooding from an area of special flood hazards. Complete engineering analyses must be submitted in support of each section of this form.
Complete the information regarding any structural flood control measures. In addition, complete and submit the Riverine Structure Form 3 and an Operation and Maintenance Plan with the revision request. The Operation and Maintenance Plan may be submitted when requesting a Conditional Letter of Map Revision, but it is not required. However, it will be required after construction is complete and a revision to the Flood Insurance Rate Map is requested.
Submit a certified topographic map showing the information indicated in the Mapping Requirements section of the form. This includes the boundaries of the alluvial fan landform, a delineation of the active and inactive portions of the alluvial fan landform, the revised 1%-annual-chance floodplain boundaries and the correct alignment of all structural features. In addition, submit a copy of the effective Flood Insurance Rate Map which is annotated to show the revised 1%-annual-chance floodplain boundaries.
